ACR Health is a not-for-profit, community-based organization providing a range of support services to individuals with chronic diseases, including HIV/AIDS, diabetes, heart disease, obesity, asthma, substance use disorders, and serious mental illnesses, and targeted prevention and sexual health services to individuals, from youth through adulthood with the goal of positive health outcomes. We are currently hiring for an Housing Retention Specialist in our Syracuse office.

    1. Incorporate best practices regarding confidentiality into all job duties and communications in accordance with Article 27-F, ACR Health’s policies and procedures and other applicable regulations. Protect agency data in accordance with confidentiality procedures and protocols. Observe and abide by the HIV Confidentiality Law.
    2. Complete and regularly update Housing Risk Assessment.
    3. Initiate and complete the process to secure decent, affordable permanent housing.
    4. Assist with completing paperwork associated with securing recertification for housing related benefits, income maintenance, and essential housing needs.
    5. Assist clients to secure services that maximize their tenure in housing; increase their ability to maintain their household and finances, independently perform activities of daily living, uphold the terms of their lease; prevent lease violations; intervene and mitigate crisis situations; maximize tenant safety and security; and prevent avoidable evictions
    6. Communicate, engage, advocate on behalf of tenants and mediate conflicts with landlords.
    7. Maintain an ongoing awareness of clients’ medical/social conditions through monthly home visits, regular communication with the client and engaged service providers, and through quarterly reassessments.
    8. Develop individual service plans in conjunction with the client, Care Manager, when one is assigned, and other related service providers.
    9. Conduct multidisciplinary case conferences and coordinate services and referrals with services and clinical providers that facilitate a client’s engagement and retention in care.
    10. Maintain complete and accurate statistical information; prepare all required monthly and program reports as assigned; ensure thorough, up to date and complete client records.
    11. Complete/submit all required documentation within designated time frames.
    12. Participate in agency case conferences as scheduled.
    13. Re-evaluate care plan/conduct case conferences on a regular basis to ensure continuity of services.
    14. Provide crisis intervention when appropriate.
    15. Maintain working knowledge of Medicaid Redesign Team program regulations, policies, and procedures.
    16. Attend all required and recommended staff trainings and in-services.
    17. Perform other duties as assigned by Housing Coordinator, the Assistant Director of Support Services, Director of Support Services, Deputy Executive Director and/or Executive Director


    BA or BS with 2 years of experience working in the field of HIV/AIDS, behavioral health, substance abuse or other chronic illnesses. Familiarity with and experience in housing and working with at risk homelessness preferred. Possess a clear understanding of community level work and the importance of collaborating and coordinating with other organizations. Effective communication and documentation skills, as well as cultural and linguistic competence. A strong commitment to the mission and vision of ACR Health.  Travel throughout ACR Health’s nine-county service area is required.  Automobile and valid NYS driver’s license required.
