Qualifications:
- BA/BS in Public Health, Human Services or related field and at least three (3) years of professional experience in care coordination, clinical practices, public health, HIV, STI, viral hepatitis or PWUD health programs. This experience must include familiarity with Medical Opioid Use Disorder systems and programs, with at least (1) year of experience facilitating professional training programs.
- Strong analytical thinking and the ability to handle multiple tasks concurrently, excellent customer service, demonstrate compassion and empathy, and good interpersonal skills.
- Computer skills and proficiency in Microsoft Office, Electronic Health Records systems is a must.
- Good communication, documentation and writing skills.
- Reliable automobile, valid NYS driver’s license and ability to travel required.
- Demonstrated commitment to cultural humility; sensitivity to HIV/AIDS, chronic illness, LGBTTGNC and BIPOC communities, and a strong commitment to the mission, vision and values of ACR Health are essential.

Primary Responsibilities:
- Coordinate with emergency departments, correctional health facilities, urgent cares, inpatient units and community-based organizations (CBOs) within the region to link individuals being discharged from these settings to services in the community through the MATTERS platform.
- Coordinate with the local treatment organizations engaged in the MATTERS network to ensure appointment availability for streamlined access to care.
- Facilitate stakeholder meetings with providers engaged within the network.
- Lead the expansion efforts for further integration of the MATTERS platform to various agencies within the region including but not limited to hospitals, treatment organizations, EMS agencies, correctional facilities, and pharmacies.
- Arrange connections to peer support.
- Coordinate transportation to clinic appointments via MATTERS transportation voucher program.
- Provide technical assistance and troubleshooting issues as it relates to connecting individuals to care through the MATTERS platform.
- Answering questions/inquiries from current and new providers in the MATTERS Network.
- Attends and participates in all MATTERS related meetings.
- Ensure follow-up for individuals referred through the MATTERS platform (24-48 hours, 30 days, 60 days, 90 days, 180 days after initial referral).
- Coordinates with service providers to secure necessary care during a crisis, share crisis intervention and emergency information.
- Coordinates with receiving treatment organizations to ensure successful linkage and engagement in care.
- Ensures the minimum requirements of encounters/services monthly, one of which may be face to face with the patient at the health hub, are met for patient engagement and to support goals and barriers to care.
- Provides linkage/navigation and refers patients to needed services to support care plan including medical and behavioral health care, patient education, entitlement programs, self-help groups, and recovery and self-management.
- Advocates for interpretation services (as needed) during follow up engagements.
- Maintains complete, current, and accurate patient follow up charts that comply with MATTERS and DOH standards.
- Documents all patient-related encounters in a timely manner.
- Completes monthly report on MATTERS related activities.
- Enters data timely and accurately to promote patient care delivery and participates in ongoing quality improvement activities and training to improve patient experience and increase data quality, as needed.
- Maintains patient confidentiality and quality care.
- Additional duties, as required.
Classification: Hourly, Non-Exempt. 35 hour work week; evenings and weekends expected for meetings, workshops, and seminars
Salary: $23.07 – $24.75 Hourly
