ACR Health is a not-for-profit, community-based organization providing a range of support services to individuals with chronic diseases, including HIV/AIDS, diabetes, heart disease, obesity, asthma, substance use disorders, and serious mental illnesses, and targeted prevention and sexual health services to individuals, from youth through adulthood with the goal of positive health outcomes. We are currently hiring for a Prevention Health Advocate at our Syracuse NY location.

Prevention Health Advocate Responsibilities:

  • Incorporate best practices regarding confidentiality into all job duties and communications in accordance with Article 27-F and HIPAA, ACR Health policies and procedures and other applicable regulations. Protect agency data in accordance with confidentiality procedures and protocols. Observe and abide by the HIV Confidentiality Law and HIPAA.
  • Provide HIV and sexually transmitted disease screening to locate previously undiagnosed HIV and STD positive individuals and link them to care.
  • Links to acute care for presenting medical issues, preventive services, and facilitated referrals for ongoing primary care, partner services, sub-specialty medical care, and behavioral health treatment services for individuals who engage in substance use.
  • Conduct enhanced outreach in the communities of Onondaga and Oswego Counties, targeting high-risk sub-populations of PWID (people who inject drugs) to promote and facilitate Harm Reduction Syringe Exchange services, Overdose Prevention services, identify individuals at risk of contracting or transmitting HIV/Hepatitis & STI’s in and make appropriate referrals for testing and treatment services. Outreach-specific activities include:
  • Provide mobile Syringe Exchange and Harm Reduction services in Oswego County twice a week, working closely with Community Specialist and Public Health officials to address drug, viral and infectious epidemics
  • Dissemination of health education, agency/program promotion and distribution of informational brochures
  • Promotion and distribution of Syringe Exchange services, Expanded Syringe Access Program, Overdose Prevention training materials, and Safer sex education with supplies in Oswego County
  • Targeted client recruitment, outreaching in areas where drug use activities may occur
  • Provide direct access to safe, sterile injection equipment and safe sharps disposal
  • Refer individuals in need of services to internal programs and external providers.
  • Provide and implement strategies with PWUD to reduce behaviors which place them or their partners at risk for contracting HIV/Hepatitis by providing harm reduction counseling and education, information on the Expanded Syringe Access Program (ESAP)
  • Provide and implement strategies with PWUD to reduce behaviors which place them or others in their networks at risk of drug overdose fatalities by providing overdose prevention education, counseling and alternative safer measures.
  • Maintain accurate client and services records in accordance with agency and SEP policies and procedures.
  • Prepare required reports and supporting data which relate to specific department activities and as assigned by the Director of the Syringe Exchange Programs.
  • Identify and develop networking relationships with community based organizations and coalitions in Oswego County.
  • Assist with the general syringe exchange program operations, including program coverage, special projects and general agency/programs initiatives.
  • Participate in appropriate and applicable training/professional development opportunities for a minimum of 24 hours of curriculum-based training annually.
  • Perform other duties as assigned by the Director of Prevention Services, Deputy Executive Directors and/or Executive Director.

Prevention Health Advocate Qualifications:

  • High School Diploma/GED or higher and lived experience with the capability to identifiably connect with the population of people who use drugs or 2 years’ experience working in non-profit public health agency providing direct client related services to diverse populations (e.g., people who use drugs, people of different ethnic backgrounds, MSM, LGBTQ, etc.)
  • Basic knowledge about HIV/AIDS, sexually transmitted infections and viral hepatitis, and experience working with people who use drugs.
  • Experience delivering stigma-free, non-judgmental and culturally-competent services.
  • Automobile with insurance and valid NYS driver’s license required, must be willing to travel between Onondaga and Oswego Counties twice a week.
  • Sensitivity to HIV/AIDS, chronic illness, LGTBQ issues, PWUD and a strong commitment to the mission, vision, and values of ACR Health are essential.
