Job Title: Peer Navigator
Location: Syracuse Office – 627 West Genesee Street
Salary: $21.00 – $23.00/hourly
Classification: Non-Exempt, full-time, thirty-five-hour (35.0) week; occasional evenings and weekends required for meetings, workshops, conferences, and seminars.

Position Summary: The Peer Navigator provides individualized and group education and support to clients, families, and friends affected by HIV. This includes guidance on medical and medication issues, benefits eligibility, and the continuum of care, helping clients maintain motivation to improve health and well-being.
Minimum Qualifications
Education and Experience:
- High School diploma, GED, or equivalent.
- Reflective of the communities served, including but not limited to bilingual individuals, people living with HIV/AIDS or related conditions, African American, Latino, LGBTQ, persons in recovery, or community members.
- Familiarity with HIV primary care, mental health, substance use, and related services within ACR Health’s Ryan White service region.
- Understanding of community-based work and a collaborative approach to coordination with individuals and organizations.
Skills and Competencies:
- Strong written and verbal communication and documentation skills.
- Cultural and linguistic competence with experience providing stigma-free, non-judgmental, culturally responsive services.
- Proficient in email management and Microsoft Office Suite; database experience preferred.
- Sensitivity to HIV/AIDS, chronic illnesses, LGBTQI+ issues, and BIPOC populations.
- Commitment to ACR Health’s mission, vision, and values with strict adherence to HIPAA guidelines.
Other Requirements:
- Valid New York State driver’s license and access to a reliable, insured vehicle for work-related travel and client transportation.
- Personal lived experience with HIV required to provide empathetic, informed peer support.
Primary Responsibilities
- Incorporate best practices regarding confidentiality consistent with Article 27-F, HIPAA, ACR Health policies, and other regulations. Protect agency data from confidentiality protocols.
- Conduct outreach and engagement to connect and retain clients in care.
- Support clients’ self-advocacy and self-management efforts.
- Accompany clients to clinical and supportive service appointments.
- Assist during Non-Medical Case Management (NMCM) interventions and educational sessions.
- Collaborate with Ryan White Part B Support Specialists to identify and provide appropriate health education for clients.
- Work with Ryan White Part B Program staff to ensure holistic client care.
- Assist Support Specialists with education group setup and facilitation.
- Ensure client adherence to Ryan White Program policies and procedures.
- Coordinate community activities with the Ryan White Program Manager.
- Communicate timely and effectively with AIDS Institute staff.
- Maintain complete and accurate client records and prepare required reports.
- Stay informed of Ryan White Part B Program regulations, policies, and procedures.
- Attend all required meetings and training.
- Perform other duties as assigned by management.
