Job Title: Youth Housing Case Manager (Temporary Position – through December 31, 2025)

 Location: Utica – 287 West Genesee Street

Salary: $22.00 – $23.10 Hourly

 Classification: Non-Exempt, full-time, thirty-five-hour (35.0) week; temporary position through December 31, 2025; occasional evenings and weekends required for meetings, workshops, conferences, and seminars.

 Position Description: The Housing Specialist works directly with individuals and families facing housing instability, assisting them in finding and securing appropriate, affordable housing. This includes conducting needs assessments, identifying available options, navigating application processes, advocating for clients, and connecting them with necessary support services to help maintain stable housing.

 Minimum Qualifications :

  1. Associate’s degree in health, human services, or a related field; OR three years’ experience providing direct client services in a nonprofit social service agency serving diverse populations (e.g., persons of color, LGBTQI+ individuals, people who use drugs, individuals with serious mental illness, those experiencing homelessness, or chronic illness management).
  2. Preferred: Experience in housing services and working with individuals at risk of or experiencing homelessness.
  3. Strong understanding of community-based work, including effective collaboration with internal teams and external agencies.
  4. Demonstrated cultural and linguistic competence, with sensitivity and commitment to serving individuals affected by HIV/AIDS, chronic illness, LGBTQ+ individuals, communities of color, people who use drugs, and other marginalized groups, along with a strong dedication to the mission, vision, and values of ACR Health.
  5. Effective communication, documentation and advocacy skills.
  6. Access to a reliable and insured vehicle is required for this position along with a valid NYS driver’s license for travel purposes.

 

Primary Responsibilities:

 

  1. Maintain strict confidentiality in accordance with Article 27-F, HIPAA, and ACR Health policies.
  2. Complete and regularly updated Housing Risk Assessments.
  3. Facilitate access to safe, affordable permanent housing through ACR Health’s Rapid Rehousing program for LGBTQ+ youth.
  4. Assist clients with paperwork for housing recertification, income maintenance, and essential housing needs.
  5. Support clients in maintaining housing stability by:
    • Enhancing their ability to manage finances and daily living activities.
    • Preventing lease violations and avoidable evictions.
    • Mediating landlord-tenant conflicts.
    • Ensuring tenant safety and crisis intervention when needed.
    • Conduct monthly home visits and maintain regular communication with clients and service providers.
    • Develop and update individualized service plans in collaboration with clients and relevant providers.
  6. Maintain accurate and up-to-date client records and statistical data.
  7. Submit all required documentation within designated timeframes.
  8. Participate in agency case conferences and regular care plan evaluations.
  9. Stay informed on HUD Rapid Rehousing program policies and procedures.
  10. Attend required staff training, in-services, and monthly coalition meetings (e.g., Coordinated Entry Workgroup, Operations Committee).
  11. Perform additional duties as assigned by the Director of Family, Education, & Support Services, Chief Programs Officer, or Chief Executive Officer.
