The Collaborations for Health Program is a collaborative approach to healthcare. ACR Health has partnered with St. Camillus, CNY Services, Christopher Community, and Upstate to provide comprehensive services to individuals hospitalized with substance use related injuries. ACR Health’s Transitional Care Manager assists clients enrolled in the Collaborations for Health Program with the following:
- Care Planning
- Housing Search
- Linkage to substance use treatment
- Transportation
- Discharge planning
