Salary:  $58,000 – $65,000

CLASSIFICATION: Exempt. Thirty-five-hour (35.00) week; evenings and weekends expected for meetings, workshops, and seminars.

Qualifications:

  1. Bachelor’s degree in human services, social work, education, or a related field with five (5) years’ experience working with qualifying populations (people experiencing homelessness, people with HIV, individuals in active substance use, BIPOC, and/or LGBTQI+ individuals).
  2. Two+ years of supervisory experience, preferably in a non-profit setting.
  3. Two+ years working in housing services or case management, with experience addressing homelessness or housing vulnerability.
  4. Possess an understanding of community level work and the importance of collaboration.
  5. Good communication, documentation, and organizational skills. Computer literacy, including email management and Microsoft Suite use required. Database experience preferred.
  6. A valid driver’s license that will pass DMV screening is required.
  7. Access to a reliable, registered & insured vehicle for travel throughout a multi-county area in the Central and Northern NY regions is required.
  8. Sensitivity to HIV/AIDS, LGBTQI+ concerns, chronic illness, needs related to substance use, acceptance of the harm reduction philosophy, and a strong commitment to the mission, vision, and values of ACR health are essential.

Primary Responsibilities:

  1. Incorporate best practices regarding confidentiality into all job duties and communications in accordance with Article 27-F HIV Confidentiality Law, HIPAA Federal Privacy Rules, and 42 CFR Part 2 regulations for protected health information related to substance use.
  2. Observe ACR Health policies and procedures, and other relevant regulations for client and agency data security. Protect agency data in accordance with requirements.
  3. Conduct annual home inspections, verify income, conduct eligibility screenings (for medical diagnoses), assist with any additional eligibility verifications for enrolled clients.
  4. Provides care coordination and necessary referrals to assist with addressing enrolled client’s health, wellness, care engagement, and housing needs.
  5. Provide oversight of program staff under assigned contracts, including, but not limited to: HOPWA, DOH Medicaid Redesign Team, RWB Housing, Rapid Rehousing, etc.
  6. Coordinate day to day duties, task assignments, and team check-ins.
  7. Provide regular group and individual supervision to program staff. Responsible for the direct supervision of program staff under assigned contracts, as well as oversight of data collection, required reporting, and quality assurance for all program contracts.
  8. Complete program enrollments, intake/screening processes, review of program applications, assessment, care planning, and case management activities as required.
  9. Ensure documentation of eligibility, income verification, quarterly screenings, and annual inspections/reassessments are completed by team members for all enrolled individuals.
  10. Conduct supervisory review of program enrollments, intakes, SDOH screenings, program applications, assessments, documentation of eligibility, care plans, and charts within 30 days of enrollment, 15 days of closure, and audit 10% of overall charts quarterly.
  11. Assist with completing paperwork associated with securing recertification for housing, applications for financial assistance and/or benefits, income maintenance, food security needs, and other essential housing or utility needs for housing clients not engaged in case management services.
  12. Assist clients with the skills and resources to maximize their tenure in housing, increase their ability to maintain their household and finances, independently perform activities of daily living, uphold the terms of their lease, understand tenant’s rights & responsibilities,
  13. Provide crisis intervention, as appropriate, to maximize tenant safety and security, prevent avoidable evictions, and maintain landlord relationships.
  14. Communicate, engage, advocate on behalf of tenants & mediate conflicts with landlords.
  15. Complete Housing Assessments and update as required, with related service plans in conjunction with client, Health Homes CM and/or RW team (when applicable).
  16. Conduct multidisciplinary case conferences and coordinate services and referrals with services and clinical providers that facilitate a client’s engagement and retention in care.
  17. Maintain contact with landlords to preserve positive relationships and address concerns.
  18. Maintain quality assurance of all assigned programs as outlined by the contractor and policies and procedures manuals. Maintain complete and accurate statistical information.
  19. Prepare all required monthly, quarterly, and annual program reports as assigned, as well as assisting with CFAs and providing input on grant applications.
  20. Maintain working knowledge of housing program regulations, policies, and procedures.
  21. Maintain working knowledge or federal, state, local and internal policies, procedures, rules, and regulations relating to housing program contracts.
  22. Participate in ongoing community service meetings/committees both locally and regionally to promote support services programs.
  23. Attend all meetings and trainings. Maintain and enhance ongoing and effective collaboration with all other internal ACR Health programs.
  24. Attend and actively participate in monthly Housing and Homelessness Coalition meetings and sub-committees, including General HHC, HMIS Administrator and Coordinated Entry workgroup meetings. Attend relevant CoC meetings throughout the region.
  25. Communicate and collaborate with Contract Managers and respond in a timely and appropriate manner to all requests.
  26. Participate as a culturally competent member of ACR Health, which includes having the capacity to function effectively as an individual within the context of cultural beliefs, behaviors, and needs presented by clients and their communities.
  27. Work in a cooperative, collegial style with all other organizational personnel. Conduct oneself in a professional manner as an agency employee and representative.
  28. Perform other duties as assigned by the Director of Family, Education & Support Services, Chief Program Officer and/or Chief Executive Officer.
